Fully Remote | Monday-Friday, Business Hours | No Weekends or Holidays
We're seeking a licensed Behavioral Health professional to join our remote care management team. In this role, you'll use your clinical expertise to support members with complex behavioral and physical health needs, helping them access the right care at the right time-while making a meaningful impact from home.
What You'll Do
- Conduct comprehensive behavioral and physical health assessments using clinical tools and data review
- Develop and coordinate individualized care plans aligned with member benefits and available resources
- Collaborate closely with providers and internal teams to ensure seamless, cost‑effective care
- Provide crisis intervention and follow‑up support for members experiencing behavioral health or medical crises
- Use Motivational Interviewing and advanced clinical judgment to reduce risk factors and improve outcomes
- Coordinate psychosocial wraparound services and community resources
What You Bring - Required
- Master's degree in Social Work or Counseling
- Active, unrestricted independent clinical license in your state (LCSW Licensed Clinical Social Worker, LISW Licensed, Independent Social Worker, LCPC Licensed Clinical Professional Counselor, LPC Licensed Professional Counselor, LP Licensed Psychologist, LMFT Licensed Marriage and Family Therapist, LMHC Licensed Mental Health Counselor)
- 3+ years of post‑master's direct clinical experience (hospital, outpatient, ambulatory, or similar setting)
- 3+ years of experience with mental health and substance use disorders
- Comfort providing care telephonically while documenting in real time
- Strong Motivational Interviewing and technology skills
Preferred
- Crisis intervention experience
- Managed care or utilization review background
- Case management and discharge planning experience

Pay Range
The typical pay range for this role is:
$60,522.00 - $129,615.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
